Randomised controlled trial of the short-term effects of osmotic-release oral system methylphenidate on symptoms and behavioural outcomes in young male prisoners with attention deficit hyperactivity disorder: CIAO-II study

Background
Research has shown that 20–30% of prisoners meet the diagnostic criteria for attention-deficit hyperactivity disorder (ADHD). Methylphenidate reduces ADHD symptoms, but effects in prisoners are uncertain because of comorbid mental health and substance use disorders.
AimsTo estimate the efficacy of an osmotic-release oral system methylphenidate (OROS-methylphenidate) in reducing ADHD symptoms in young adult prisoners with ADHD.
MethodWe conducted an 8-week parallel-arm, double-blind, randomised placebo-controlled trial of OROS-methylphenidate versus placebo in male prisoners (aged 16–25 years) meeting the DSM-5 criteria for ADHD. Primary outcome was ADHD symptoms at 8 weeks, using the investigator-rated Connors Adult ADHD Rating Scale (CAARS-O). Thirteen secondary outcomes were measured, including emotional dysregulation, mind wandering, violent attitudes, mental health symptoms, and prison officer and educational staff ratings of behaviour and aggression.
ResultsIn the OROS-methylphenidate arm, mean CAARS-O score at 8 weeks was estimated to be reduced by 0.57 points relative to the placebo arm (95% CI −2.41 to 3.56), and non-significant. The responder rate, defined as a 20% reduction in CAARS-O score, was 48.3% for the OROS-methylphenidate arm and 47.9% for the placebo arm. No statistically significant trial arm differences were detected for any of the secondary outcomes. Mean final titrated dose was 53.8 mg in the OROS-methylphenidate arm.
ConclusionsADHD symptoms did not respond to OROS-methylphenidate in young adult prisoners. The findings do not support routine treatment with OROS-methylphenidate in this population. Further research is needed to evaluate effects of higher average dosing and adherence to treatment, multi-modal treatments and preventative interventions in the community.

While the summit of the Antarctic Plateau has long been expected to harbor the best ground-based sites for terahertz (THz) frequency astronomical investigations, it is only recently that direct observations of exceptional THz atmospheric transmission and stability have been obtained. These observations, in combination with recent technological advancements in astronomical instrumentation and autonomous field platforms, make the recognition and realization of terahertz observatories on the high plateau feasible and timely. Here, we will explore the context of terahertz astronomy in the era of Herschel, and the crucial role that observatories on the Antarctic Plateau can play. We explore the important scientific questions to which observations from this unique environment may be most productively applied. We examine the importance and complementarity of Antarctic THz astronomy in the light of contemporary facilities such as ALMA, CCAT, SOFIA and (U)LDB ballooning. Finally, building from the roots of THz facilities in Antarctica to present efforts, we broadly highlight future facilities that will exploit the unique advantages of the Polar Plateau and provide a meaningful, lasting astrophysical legacy.
Leptospirosis following a major flood in Central Queensland, Australia

Throughout December 2010 and January 2011, Queensland experienced widespread flooding due to unusually protracted and heavy rainfalls. In mid-January 2011, four individuals from a small community in Central Queensland were hospitalized with leptospirosis. A further five cases were subsequently identified from around Central Queensland, bringing the total to nine. Microscopic agglutination testing found that serovar Arborea (Leptospira borgpetersenii serovar Arborea) was presumptively responsible for leptospirosis in seven of nine confirmed cases. Serovars Hardjo and Australis were identified in samples from two remaining cases. All cases had exposure to flood water. No single exposure source was identified. This is the first reported outbreak of leptospirosis in Central Queensland and the first report of leptospirosis cases associated with flood water inundation in Queensland. Public health authorities should continue to promote awareness of leptospirosis in flood-affected populations. Healthcare providers must maintain a high level of suspicion for leptospirosis during and after flood events.

Comparative ultrastructural features of the ovary and vitellogenesis have been described for six shallow water and four bathyal species of sea cucumbers representing four major holothuroid orders. Ovarian structure is similar in all ten species except for features of the peritoneal cells of the outer layer and the follicular inner epithelial cells surrounding the developing oocytes. The peritoneal cells vary from monociliated squamous or cuboidal cells to large columnar cells. Ultrastructural evidence suggests that these cells might be capable of incorporating materials from the perivisceral coelom. The follicular inner epithelial cells of two deep-sea species resemble podocytes, a feature previously unre-ported in holothuroid ovaries. It is suggested that these cells function to increase nutrient exchange between the genital haemal sinus and the oocyte during vitellogenesis. In all ten species, the oocytes appear to participate in yolk synthesis through the interaction of the Golgi complex and rough endoplasmic reticulum. The similarity in the ultrastructural features of vitellogenesis suggests that the process of yolk synthesis has been highly conserved in holothuroids. Endocytotic activity was detected in seven of ten species but it is uncertain if this is directly related to vitellogenesis. Cilia and intracellular structures resembling striated ciliary rootlets were observed in the oocytes of four of the ten species studied. The significance of this finding is unclear but could indicate that germ cells have a somatic cell origin.
A New Species of Poecilosclerid Sponge (Porifera) from Bathyal Methane Seeps in the Gulf Of Mexico

A new species of poecilosclerid sponge from methane seeps on the Louisiana Slope, Gulf of Mexico, USA is described. This thickly encrusting sponge grows around vestimentiferan tubes and has a very patchy distribution. The skeletal structure and the spicule set match those characterizing the genus Ectyomyxilla. The new species, Ectyomyxilla methanophila sp. nov., is distinguished from the remaining species in the genus by the structure of its ectosomal skeleton, where small acanthostyles are never present, and by its choanosomal skeleton, which is barely reticulate and has echinated main tracts. The small size of the choanocyte chambers suggests occurrence of nutritional benefits derived from association with chemoautotrophic symbiotic bacteria. However, the presence of oscules, ostioles, aquiferous canals and choanocyte chambers indicates that this sponge also relies partly on filtration for its nutrition. This is the first record of the genus Ectyomyxilla in the northern hemisphere, and the first non-carnivorous demosponge known to exploit methane-rich marine environments.
Vertical Distribution Correlates With Pressure Tolerances of Early Embryos in The Deep-Sea Asteroid Plutonaster Bifrons

The astropectinid asteroid Plutonaster bifrons (Wyville Thomson) occurs on the continental slope of the north-east Atlantic between 1000 and 2500 m depths. As in most deepsea animals, the factors limiting bathymetric distribution of this species are unknown. Eggs were fertilized in vitro and incubated through the early embryonic cleavage stages at pressures that correspond to depths from 0 to 3000 m. The highest percentage of normal development occurred near the peak of the species distribution (2000 m), and virtually no normal development occurred at a pressure corresponding to 3000 m depth. Develop-mental rate was retarded at pressures higher and lower than those found near 2000 m. These experiments indicate that embryonic pressure tolerances could determine both the upper and lower bathymetric limits of distribution for this species.
